Review of progress of the second Midori Ward community-based welfare health plan "Midorinowa, support love plan" (from 2011 to 2015) and basic document in the third plan formulation (from 2016 to 2020) We carried out inhabitant of a ward questionnaire to do. The results of the survey are disclosed.

Under the basic philosophy of "Aiming for a Midori Ward where everyone can continue to live with peace of mind," Midori Ward is promoting the Midori Ward Community Welfare and Health Plan "Midorinowa Support Ai Plan" based on the basic philosophy of "creating a community where everyone can play a leading role and support together."
We carried out inhabitant of a ward questionnaire to reflect on progress situation of the second phase (from 2011 to 2015) and to assume basic document in the third plan development (from 2016 to 2020) .

Overview of the questionnaire

Survey period: From Thursday, August 21, 2014 to Monday, September 8
Survey target: 4,500 Midori Ward residents over 20 years old (randomly extracted)
Survey method: Distribution and collection of questionnaires by mail
Number of collections: 1,301 cases Collection rate 28.9% Valid votes 1,301 cases
Survey contents (total of 30 questions): (1) Overview of respondents; (2) Questions by area in line with the five keywords of the basic goals of the “Midori nowa / Support Ai Plan”; (3) Questions on Health and Welfare in general
