How to apply

If you would like to participate, please send the application form (Form A) for Midori Ward Health and Welfare Center Application Organizations to Health and Welfare Division Midori Ward by e-mail.
There is a possibility that the parking lot at Midori Ward Office may be full on the day of the briefing, so please use public transportation if possible. There is a charge for parking at the Midori Ward Office.

On the day, application guidelines and application documents will not be distributed, so please bring your own.

Questions regarding the contents of the application guidelines will be accepted as follows.

Please send the “Questionnaire (Form B)” to Midori Ward Health and Welfare Division Business Planning Officer by e-mail. Please note that we cannot respond to questions over the phone.

Please submit to the Midori Ward Health and Welfare Division Business Planning Section (39th window on the 3rd floor of Midori Ward Office) by mail or by sending records (registered mail, etc.) (must arrive within the application period).
