If you would like to participate, please send the application form for Midori Ward Health and Welfare Center Application Form (Form 12) to the Health and Welfare Division Business Planning Section, Midori Ward.
Please note that the parking lot of Midori Ward government office may be full on the day of the briefing, so please use public transportation if possible. Parking is charged at the Midori Ward Office.

On the day, application guidelines and application documents will not be distributed, so please bring your own.

Questions regarding the contents of the application guidelines will be accepted as follows.

Please submit the “Questionnaire (Form 13)” to the Health and Welfare Division Business Planning Section, Midori Ward. Please note that we will not be able to respond to telephone questions.

On Wednesday, February 12, 2020 (planned), the answer will be announced on this website.
