(4)How to apply
If you would like to participate, please send the “Application Form for Briefing Session for Midori Ward Health and Welfare Center Application Organizations (Form 12)” to Midori Ward Health and Welfare Division Business Planning Officer.
There is a possibility that the parking lot at Midori Ward Office may be full on the day of the briefing, so please use public transportation if possible. There is a charge for parking at the Midori Ward Office.

On the day, we will not distribute the application guidelines and application documents, so please bring your own.

Questions regarding the contents of the application guidelines will be accepted as follows.

(2)Submission method
Please submit the “Questionnaire (Form 13)” to Midori Ward Health and Welfare Division Business Planning Officer by e-mail. Please note that we cannot respond to questions over the phone.
※The subject of the e-mail should be "[Question] About the 4th Midori Ward Welfare and Health Activity Base Designation Management Selection Committee".

On Wednesday, February 12, 2020 (planned), the answer will be announced on this website.

Please bring your application documents or submit them to the Midori Ward Health and Welfare Division Business Planning Section (registered registered mail, etc.) (must arrive within the application period).

※Of the application documents, the balance sheet, property inventory of the last three years up to the previous fiscal year of the fiscal year to which the date of submitting the designated application belongs, “h List of Names of Officers (Form 6)” and “m Please note that the profit and loss statement and fund balance statement, etc. are also required to submit Excel data stored on CD-R.
