If you would like to participate, please send the “Yamashita Community Care Plaza Field Tour / Application Briefing Application Form” (Form 13) to Midori Ward Health and Welfare Division Business Planning Officer via e-mail.

※On the day, we will not distribute the application guidelines and application documents, so please bring your own.
※There is no parking available. Please use public transportation.

If you would like to participate, please send the “Yamashita Community Care Plaza Field Tour / Application Briefing Application Form” (Form 13) to Midori Ward Health and Welfare Division Business Planning Officer via e-mail.
There is a possibility that the parking lot at Midori Ward Office may be full on the day of the briefing, so please use public transportation if possible. There is a charge for parking at the Midori Ward Office.

Please submit the “Questionnaire” (Form 14) to Midori Ward Health and Welfare Division Business Planning Officer by e-mail.
Please note that we cannot respond to inquiries over the phone.

Please bring your application documents or submit them to the Midori Ward Health and Welfare Division Business Planning Section (registered registered mail, etc.) (must arrive within the application period).

※Among the application documents, "Ki Officers' Name List (Form 5)" and "Balance sheet, property inventory, profit and loss statement, fund balance statement, and file data for the fiscal year for the last three years" Please note that it is also mandatory to submit Excel data stored on CD-R.
