birth registration form death registration/death notification form

When a foreigner living in Japan gives birth in Japan, the following items must be done.

1. Submitted by birth registration form
2. A child's status of residence
3. Application to the Government of Japan

Please notify the Family Registry Division window of the municipality where you have registered as a resident.

1. birth registration form and birth certificate (birth registration form paper will be given with birth certificate if given birth at a medical institution)
2. Mother and Child Handbook
3. Parent's passport (only if the parent's nationality is requested on the receipt certificate)

1. Katakana (please add Roman letters)
2. Kanji (for countries whose names are written in kanji under the law)

death registration/death notification form shall notify you within 7 days from the date you become aware of the death.
※If you are a foreigner married to a Japanese spouse and your Japanese spouse has died, you will need to change your status of residence. Please contact us.

1. Relatives of living together (not living with relatives who do not live together)
2. Other cohabitants
3. Landlord, landlord, house or land caretaker

• death registration/death notification form (receive the form with the doctor's death certificate (postmortem certificate) of the doctor who attended the death)
• Fire burial permission application (available at the Family Registry Division window)

※We submit fire burial permission application to window together with death registration/death notification form. Fill out the fire burial permission application form with the deceased's nationality, current address, crematorium, etc. If there is no defect in the documents, a fire burial permit will be issued on the spot.
