Here is an example of payment for households under the age of 70.

If the co-payment paid by the same person to the same medical institution in the same month exceeds the limit, the surplus will be paid.
If the same household pays more than 21,000 yen co-payment twice or more in the same month, the sum exceeding them will be paid in excess of the limit.
If high medical costs receives 4 or more payments from the same household within the past 12 months, the 4th and subsequent payments will be made to the high medical costs.
