Basic knowledge of infertility
1.What is infertility?
A healthy man and woman who wish to become pregnant lives married without contraception, but does not become pregnant after a certain period (about one year). In Japan, it is said that there are 5.5 to 1 in 6 couples who have been tested or treated for infertility.
2.Cause
Men and women are responsible for not having pregnancy.
[Cause of Women]
・Ovulation failure: It is difficult to cause ovulation, such as when ovum does not grow well.
・Fallopian tube failure: An obstacle to the "ovarian tube" that connects the ovaries and the uterus, such as clogged or narrow egg tubes.
・Landing failure: Endometriosis, polyps, uterine fibroids, etc., which are difficult to land.
・Transmission of sperm in the cervical tube: Anti-sergic antibody with less cervical mucus ※Etc.
※Antiseptic antibodies are antibodies that determine sperm as foreign substances from the outside and remove them. If there is such an antibody, sperm will be rejected.
[Cause of Men]
・Differential dysfunction: It means that sperm cannot be made well, such as a small number of sperm, poor movement, and a high malformation rate.
・Sexual dysfunction: Erection and ejaculation are not good. It means that sexual intercourse cannot be done well.
・Road passing obstruction: The path of sperm is narrow or clogged.
3 If you think it's infertility as soon as possible
The secretion of female hormones, which are closely related to pregnancy, changes with age. The secretion of female hormones peaks in their 20s and early 30s. Also, after the age of 40, the secretion of female hormones decreases.
ART(Assisted Reproductive Technology): Reproductive assistance medical technology, ET (Embryo Transfer): Embryo transfer
Quoted from the Japan Society of Obstetrics and Gynecology ART Data Book 2019 (http://plaza.umin.ac.jp/~jsog-art/2019data_202107.pptx (external site)))
If you receive fertility treatment and receive reproductive supplementation, you can see from the figure above that the outcome is related to your age.
If you want to become pregnant, but if sexual intercourse does not go well, you may be worried about it, etc., if you feel that you are not quite pregnant, consult a doctor or test as soon as possible.
If you are not comfortable with the examination or consultation, you can also consult with the Fertility Consultation Center in each prefecture or core city.
In Yokohama City, we accept consultations on infertility and infertility. Please feel free to contact us.

4 Test for infertility
・Female hormones are secreted according to menstrual cycles, so tests are performed according to that time.
・By adding the basal body temperature, you can see the condition of hormone secretion.
・There are also tests that can be done at any time, regardless of the body cycle.
Interview, internal examination, ultrasound, chlamydia antigen antibody test, sexual disease test, uterine cancer (cervical cancer, body cancer), basal body temperature, antisem antibody test
5 Treatment of infertility Steps of fertility treatment for pregnancy and childbirth
~ 1st stage ~ Try to do by ourselves
First, record the basal body temperature for 2-3 months, predict the ovulation based on the basal body temperature, and have sexual intercourse. It is said that pregnancy is more likely from 2 days before the ovulation date to immediately after the ovulation.
The basal body temperature is just before the body temperature rises, the vasacity decreases and becomes transparent, and the amount increases. You can also purchase and use an ovulation test drug at a pharmacy.
~ Phase 2 ~ Timing therapy
Medical institutions conduct ultrasonography and hormonal tests, predict the date of ovulation, and receive guidance on the date of sexual intercourse according to the timing of ovulation.
~3rd stage ~ artificial fertilization
This is a treatment that uses a dedicated catheter to deliver the washed and concentrated sperm into the uterus according to the timing of ovulation, and expects a natural pregnancy.
It occurs when married couples with unexplained infertility, sexual disorder, partner is deficiency sperm asymptomatic, etc.
[General flow of artificial insemination]
①I'll predict the ovulation date.
The ovulation date is predicted by referring to the basal body temperature, cervical mucus, ultrasound, and hormone values.
Inducing agents (drugs and injections) may be used to grow ovum.
②The sperm of the partner is collected according to the woman's ovulation day.
③Treatment of sperm (washing and concentrating).
④Use a catheter to put sperm into the uterus.
~ Stage 4 ~ Reproductive assistance (in vitro fertilization / microinsemination)
Treatment if pregnancy is not established by the third stage, or if both fallopian tubes are blocked or if the sperm is extremely low.
◇In vitro fertilization
The ovum is removed before ovulation, sperm is applied to ovum outside the body, fertilized, and the fertilized egg is returned to the uterus.
◇Microinsemination
In vitro fertilization is a treatment in which human sperm is directly injected into ovum when the number of sperm is small.
[General flow of in vitro fertilization and microinsemination]
①It uses an ovulation derivative to stimulate the ovaries and grow multiple ovarians. The follicles are stabbed from the vagina wall and collected together with the follicle fluid by suctioning ovum.
②The sperm is collected at the same time.
③In vitro fertilization sprinkles sperm on ovum and fertilizes it. Microinsemination injects sperm directly into the egg cell type under a microscope and fertilizes it.
④They grow fertilized eggs in the culture medium. (Culture)
⑤Using a catheter, the grown fertilized egg is returned to the uterus (embryo transplantation). Replenish the yellow hormone to make it easier for the fertilized egg to land.
⑥The pregnancy is judged about two weeks after the embryo transfer.
6 Life of those who are receiving fertility treatment
Infertility treatment is not the same.
There may also be those who want to have a prospect of how much is related to daily life, such as the number of consultations.
For reference, I would like to introduce the status of consultation by some people.
