Overview

For those who can expect treatment, training, and assistance effects by using training equipment, self-help tools, and assistance tools (hereinafter referred to as "equipment, etc."), we will support a part of the purchase cost of equipment.

**●Outline of Yokohama City Training and Assistance Equipment Subsidy Project**　※For details, please refer to the "Guide to Yokohama City Training and Assistance Equipment Subsidy Project".
Target person	Children with disabilities living in Yokohama City (limited to those under 18 years old) ※Those who are hospitalized (admission) are not eligible.
Target items	Please refer to Appendix 1 (PDF: 161KB).
Grant amount	Two-thirds of the purchase cost of equipment or the subsidy limit, whichever is lower. ※The grant limit varies depending on the item. Please refer to Appendix 1.
What you need	・Quotation for equipment, etc. ・Prescription or doctor's opinion, etc. (only if necessary according to the target item)
Precautions	・Application is required in advance. ・It cannot be used at the same time as other grant programs. ・It is not eligible if the municipal tax income percent of any of the members of the household exceeds 460,000 yen. ※１ *・Grant applications can be made only once during the fiscal year (April 1 to late February of the following year 2).**

　※If the month of application is from April to June, the municipal tax income percent will be confirmed for the previous fiscal year, and if it is from July to March of the following year.
　In addition, we calculate the following 1 to 4 from the municipal tax income percent for the relevant year.
　Please confirm each amount in special collection tax amount decision letter of advice.

　①Even if you live in a government-designated city, use the standard rate (6%) before the tax reform in 2017 is used.
　②The municipal tax income percent uses the amount before special tax credits such as housing borrowings and donation tax credits are deducted.
　③Use the calculation before the deduction of young and specific dependent relatives is abolished.
　④Yokohama City widow (husband) deduction is applicable.

Application Flow

（１） If you would like to apply for a grant (guardian of a child with a disability), please consult with the handling organization about the effects, necessity and requirements for the furtherance at the handling organization.
（２） If the necessity is determined, request the equipment purchaser to create a quote, and ask the handling organization to make an application form, quote, prescription, etc. (for necessary items
Please submit it as soon as possible from the date of creation.
（３） When Yokohama-shi examines subsidy contents and makes a furtherance decision, "subsidy decision letter of advice" is issued. At the same time, we will send a subsidy ticket from Yokohama City to the company planning to purchase equipment. Please adjust the date of receipt of the equipment etc. with the company planning to purchase equipment.
（４） Equipment, etc. You will receive equipment from the company planning to purchase equipment. At that time, please pay the copayment amount and sign the grant ticket on the date of receipt and signature. After that, the public expenditure will be paid from Yokohama City Hall to the business operator.
