About system for person with a disability and application of disability certificate

• Have the designated doctor make the prescribed medical certificate / statement of position, attach a photo, and apply.
• We hand medical certificate, statement of position of certificate of the physically disabled, notebook of love (care notebook) at ward office window.
• You can download the medical certificate of the Mental Health Welfare Handbook from the website. (It is also possible to hand over at the ward office window.)
• Medical certificate (select one of the forms depending on the printing environment.)
