Notification of The Long-term Care Insurance's Qualifications

When applying for The Long-term Care Insurance certification
Please apply to the ward office's Elderly and Disabled Support Division (11th window on the first floor).
When a person living in Yokohama celebrates his 65th birthday
There is no need to submit a report.
For those who celebrate their 65th birthday between the second day of the following month and the first day of the following month, we will send you a The Long-term Care Insurance card at the end of the month.
When a person over the age of 65 moves into Kohoku Ward from outside the city
Please report to Insurance and Pension Division National Health Insurance Section (27th floor on the second floor) of the ward office.
<Required for notification>
- Certificate of eligibility (only those who have already been certified as requiring long-term care. It is issued by the previous municipalities.
*Please submit a transfer notice of resident registration first.
When a person over the age of 65 moves into Kohoku Ward from another ward in Yokohama
Please report to Insurance and Pension Division National Health Insurance Section (27th floor on the second floor) of the ward office.

Please report to Insurance and Pension Division National Health Insurance Section (27th floor on the second floor) of the ward office.

If you have lost your The Long-term Care Insurance card
If you have lost your insured card, please notify the ward office's Insurance and Pension Division National Health Insurance Section (27th floor on the second floor). I will reissue the insured card of The Long-term Care Insurance by mail.

- Documents with name, insured number, etc., such as notices about The Long-term Care Insurance
- Identification documents (driver's license, my number card, etc.) of the person coming to the notification
- Power of Attorney (if the person coming to the notification is other than the person's family)
*If lost outside the house, be sure to notify the police as it may be abused.

If a person who already has a certificate of insured person in The Long-term Care Insurance, Yokohama has died, please notify the ward office's Insurance and Pension Division National Health Insurance Section (27th floor on the 2nd floor).

We issue identification of insured person to all first insured persons and some of the second insured persons (persons applying for certification, etc.). The insured person's card is required for the first time when applying for certification, so if you are independent, please keep it in a safe place for the time being.
When applying for certification, please bring it to the ward office's Elderly and Disabled Support Division (11th floor on the first floor). If you are certified for nursing care, you will be given the details of the certification after the second side, so please show it when you receive nursing care services.
The contents of the insured person's card must not be rewritten. Please use it so that the contents are clearly understood and not dirty.
