We issue identification of insured person for all the first insured person and part of the second insured person (person applying for certification). The insured person's card is required for the first time when applying for certification, so if you are independent, please keep it in a safe place.

When applying for certification, please bring it to the ward office's Elderly and Disabled Support Division (1st floor 11th window). When you are certified for nursing care, you will be given the contents of the certification after the second page and hand it over, so please show it when you receive nursing care services.

The contents of the insured person's card must not be rewritten. Please use it so as not to get dirty so that you can clearly see the contents.
