Notification of qualifications for The Long-term Care Insurance

When applying for The Long-term Care Insurance certification

Please apply to the ward office's Elderly and Disabled Support Division (1st floor 11th window).

When a person residing in Yokohama celebrates his 65th birthday

There is no need to submit a notification.
For those who reach their 65th birthday from the 2nd of the following month to the 1st of the following month, we will send you a The Long-term Care Insurance insured card at the end of the month.

When a person 65 years or older moves into Kohoku Ward from outside the city

Please report to Insurance and Pension Division National Health Insurance Section (27th window on the second floor) of the ward office.

<Required for notification>

  • Certificate of Eligibility (only for those who have already been certified as requiring long-term care.) It will be issued by the previous municipality. )
  • private seal of the person coming to the report

When a person 65 years or older moves into Kohoku Ward from another ward in Yokohama.

When a person over the age of 65 changed Address in Kohoku Ward

Please report to Insurance and Pension Division National Health Insurance Section (27th window on the second floor) of the ward office.

  • If you are certified as requiring long-term care, you will be issued a certificate of eligibility. Please submit it to the municipalities to which you are moving to.

When a person 65 years or older dies

Please report to Insurance and Pension Division National Health Insurance Section (27th window on the second floor) of the ward office.

  • death certificate (not required if you have already sent death registration/death notification form to the Kohoku Ward Office.)

When I lost my The Long-term Care Insurance insured card

If you have lost your insured card, please notify the ward office's Insurance and Pension Division National Health Insurance Section (27th window on the second floor). The Long-term Care Insurance's insured card will be reissued by mail.

  • Documents stating the name and insured number, etc. regarding The Long-term Care Insurance
  • Identification documents (driver's license, my number card, etc.) of the person coming to the notification
  • private seal of the person coming to the report
  • Power of Attorney (if the person coming to the notification is other than the person's family)

*If lost outside the house, be sure to notify the police as it may be misused.

No insured person's card is usually issued to the second insured person. When you need an insured person's ID, such as applying for certification, please notify the ward office's Insurance and Pension Division National Health Insurance Section (27th window on the second floor).

When a person under the age of 40 to 65 dies

If you already have an insured person's card in The Long-term Care Insurance, Yokohama City has died, please notify the ward office's Insurance and Pension Division National Health Insurance Section (27th window on the second floor).

If you already have an insured card in The Long-term Care Insurance, Yokohama City and have lost your insured card, please notify the Insurance and Pension Division National Health Insurance Section (27th window on the second floor) of the ward office. We issue identification of insured person for all the first insured person and part of the second insured person (person applying for certification). The insured person's card is required for the first time when applying for certification, so if you are independent, please keep it in a safe place.

When applying for certification, please bring it to the ward office's Elderly and Disabled Support Division (1st floor 11th window). When you are certified for nursing care, you will be given the contents of the certification after the second page and hand it over, so please show it when you receive nursing care services.

The contents of the insured person's card must not be rewritten. Please use it so as not to get dirty so that you can clearly see the contents.
