About application of certification for long-term care

1.About application of certification for nursing care, support required
If you need nursing care and use The Long-term Care Insurance services, you need to apply for certification for nursing care.
The certification of a new application takes effect not from the date of the certification result, but from the date of application.
Once you have been certified as requiring long-term care, you will need to apply for renewal before the end of the certification period to continue using The Long-term Care Insurance services. You can apply for renewal 60 days before the end date of the certification period (PDF: 65KB).
Those who are eligible for the system
- 65 years or older (first insured person)
- Those who are between the ages of 40 and 64 and have a specific illness (second insured)

Necessary items at the time of application
- The Long-term Care Insurance Insured Card (for those aged 40 to 64, medical insurance card)
- In order to ask the doctor on the application form to prepare a written opinion, you can confirm the name and location of the medical institution in family doctor, Phone number, Medical consultation fee, and doctor's full name.

※In addition to the above window, you can also request a designated home care support company or a long-term care insurance facility to submit an application.
※In the case of application at Local Elderly Care Management Center or agency application, the date on which the ward office received the application is the application date.
About application by mail
You can also apply for certification for long-term care by mail.
※However, the application date is not the entry date of the application, but the date accepted by the ward office.

2.About the flow after application for certification
About flow after application for nursing care (support required) certification
Please take a look at it.
Once the certification results are obtained, we will send you a letter of certification and a new The Long-term Care Insurance insured card.
The certification result notice and the long-term care insurance card will be sent by registered mail to the registered location of resident certificate.
※Since it is sent without transfer, it will not be transferred even if the transfer notification has been sent at the post office.
If it is difficult to receive certification results at resident certificate registered locations, we may be able to change the destination.

3.Withdrawal of Application
If the condition is not stable due to hospitalization, etc. after applying for certification for long-term care, or if you cannot visit a hospital and it is difficult to prepare a written opinion for family doctor, you can withdraw your application.
You can also withdraw your application if you have applied but are unable to use the The Long-term Care Insurance Service for the time being (such as being hospitalized with medical insurance) or if you no longer need The Long-term Care Insurance Service.

- Those who are applying for certification by The Long-term Care Insurance and have not received certification results
※Please contact The Long-term Care Insurance for the progress of the certification. At that time, we will confirm the applicant's insured number and name of The Long-term Care Insurance, so please prepare a note.
