About application of certification for long-term care

1.About application for nursing care, support required certification
In order to be in need of nursing care and to use The Long-term Care Insurance services, you must apply for certification for long-term care.
The validity of a new application will take effect not from the date of the certification result, but from the date of application.
In addition, once you are certified as requiring long-term care, you will need to apply for renewal before the end of the certification period to continue using The Long-term Care Insurance services. Renewal applications can be made 60 days before the end date of the certification period (PDF: 65KB).
Those who are subject to the system
- 65 years or older (first insured person)
- Those who are 40 to 64 years old and have a specific illness (second insured person)

What you need at the time of application
- The Long-term Care Insurance insured person's card (medical insurance card for those aged 40 to 64)
- In order to ask the doctor written on the application form, a memo to confirm the name and location of the medical institution in family doctor, Phone number, Medical consultation fee and doctor's full name.

You can also apply for certification for long-term care by mail.
※However, the application date is not the date of entry of the application form, but the date of receipt by the ward office.

- The Long-term Care Insurance insured card (for those aged 40 to 64, a copy of the medical insurance card)

2.About the flow after the certification application
About the flow after applying for nursing care (support required) certification
Please take a look at it.
When the certification result is obtained, we will send you a certification result notice and a new The Long-term Care Insurance insured card.
The certification result notice and long-term care insurance card will be sent to the resident certificate registered place by simple registered mail.
※Since it is sent without forwarding, it will not be forwarded even if a transfer notification has been sent at the post office.
If it is difficult to receive certification results at the registered location of resident certificate, you may be able to change the destination.

3.Withdrawal of application
If the condition is not stable due to hospitalization, etc. after applying for certification for long-term care, the application can be withdrawn if the condition is not stable due to hospitalization, etc., or if you cannot consult a hospital and it is difficult to create a family doctor opinion.
You can also withdraw your application if you have applied but are unable to use the The Long-term Care Insurance Service for the time being (for example, you are hospitalized with medical insurance), or if you no longer need to The Long-term Care Insurance services.
