About the end of issuance of medical treatment certificate

Issuance application of medical treatment certificate in Yokohama-shi ends on Friday, May 31, 2024.                          Details are as follows.

 Applications received will be issued sequentially from the following week.

First of all, please consider alternative documents.

 In addition to the certificate of medical treatment, the Life Insurance Association shows the following examples of documents that can be used as alternative documents that can be confirmed to have a new coronavirus infection in addition to the medical treatment certificate. As for the details, please see page (outside site) of "General Life Insurance Association".
 First of all, before applying for a certificate of medical treatment, please consider the procedure with alternative documents.


Please contact your insurance company for any documents that can be handled.

  • Those that show the results of PCR tests and antigen tests conducted at medical institutions.
  • Medical treatment statement (medical management fee includes "addition of hospitalization for patients with type 2 infectious disease patients" (including temporary treatment for outpatient care and medical fees))
  • Prescription and instruction manual containing coronatherapy drug
  • Received results (SMS, LINE, etc.) of the Health Follow-up Center (Kanagawa Prefecture Positive Registration Desk) established by local governments
  • A copy of the email exchanged between the health center and the positive person
  • Guidance sentences issued by the public health center to positive persons (describe points to note about health observation and life support)
  • Test results (excluding commercially available test kits) of test centers (other than medical institutions) that conduct PCR tests and antigen tests

Those who cannot prove medical treatment in Yokohama City

■Those who have not consulted a medical institution

 In Yokohama City, we will certify medical treatment based on the "Notification of Occurrence" regarding the new coronavirus infection submitted by medical institutions to the public health center.
 Therefore, Yokohama City cannot prove medical treatment for those who have not consulted a medical institution and who fall under the following.

  • Those who are judged to be positive by self-examination
  • Those who have been tested positive at an inspection center, etc.

 If any of the above applies, check with your insurance company and consider the procedure using alternative documents.

■Those who have submitted a notification of "voluntary medical treatment" to Kanagawa Prefecture before September 25, 2022

 For those who are found to be positive by a commercial antigen test kit or a free test in Kanagawa Prefecture, and who did not consult a medical institution and submitted a notification of "voluntary medical treatment" to Kanagawa Prefecture before September 25, 2022, Yokohama City cannot issue a medical treatment certificate. (Issuance of medical treatment certificate in Kanagawa Prefecture *.)
※However, as an exception, even if you have submitted a “Notification of Voluntary Medical Treatment” to Kanagawa Prefecture will have to apply to Yokohama City in the following cases.
[Exceptions]

  1. If you have received a positive diagnosis at a medical institution, but you have reported a “voluntary medical treatment notification”
  2. If you are receiving a food distribution service in Kanagawa Prefecture during the voluntary medical treatment period

※In Kanagawa Prefecture, the issuance of medical treatment certificates has ended as of the end of September 2023.


■Those who have been diagnosed between September 26, 1945 and May 7, 2013 and do not fall under the 4 types (see below)

※What is the four types?

  1. 65 years old or older (same as at the time of diagnosis)
  2. Pregnant women
  3. Those who need to be hospitalized
  4. Those who are at risk of illness and whose physician determines that treatment of COVID-19 infection or new oxygen administration is required

 If you have been diagnosed with a new coronavirus infection at a medical institution during this period and do not fall under the above four types, there is no need to submit a “Notification of Occurrence” from the medical institution to the public health center. Proof of medical treatment cannot be proved.

■Those who have been diagnosed on or after May 8, 2023

 Due to the change of the type of coronavirus infection from Class 2 to Class 5 under the Infectious Diseases Law on May 8, 2023, it is no longer necessary for medical institutions to submit a notification of outbreaks, so the public health center will issue a medical certificate.


 In order to avoid the tightness of fever outpatients at medical institutions, companies and schools, etc. are requested to cooperate with companies, schools, and other stakeholders to avoid requesting documents or medical certificates to prove the results of tests issued by medical institutions when employees or elementary school student are infected with COVID-19 andelementary school student. (Refer to the Ministry of Health, Labour and Welfare Office)
 Please consider the following documents to prove that you have suffered from the new coronavirus infection.


  • Medical treatment statement stating "Specific disease medical treatment management fee (less than 100 beds, medical treatment guidance) (exceptional)" (medical management fee)
  • Medical treatment statement containing "Emergency Medical Care Management Addition 1 (Admission Adjustment)" (Medical Management Fee)
  • Those that show the results (positive / negative) tests performed with commercially available test kits, etc. (including images)
  • Instructions, etc. with coronatherapy drug

  In addition, after a certain medical treatment period has passed, please cooperate not to request a negative test certificate or a healing certificate.

About proof of medical treatment

 If you meet the requirements for issuing a medical certificate and have difficulty preparing alternative documents, we will issue a medical treatment certificate.
 Please note that it will take up to one month to issue it.

    Application for mailing

     If electronic application is difficult, please prepare a reply envelope (with a stamp for the fee) and apply by "mail".
