Health Checkups Yokohama City

As one of the measures against lifestyle-related diseases prevention, which focuses on cardiovascular diseases such as heart disease and stroke, we will implement the Health Checkups once a year.

Medical System for Elderly Senior Citizens with Address in Yokohama City ※１
social security recipients who have Address in Yokohama (including those who turn 40 by the end of the fiscal year)
Those who are 40 years old or older (including those who become 40 years old by the end of the fiscal year) who have a Address in Yokohama. ※２

However, those who fall under the following are not eligible.
Those who are in nursing homes for the elderly to which The Long-term Care Insurance applies

※1 Target person of medical system insured person for elder senior citizens is person corresponding to any of the following.
　　Oh, 75 years or older (from 75th birthday)
　　(A) Those who are 65 years old or older and under 75 years old and have been certified by the Wide Area Union as having a certain degree of disability (from the date of certification)

※2 The target persons who remain in China are those who meet all of the following conditions and their spouses.
　　Oh, person born after April 2, 1911
　　B) Persons born before December 31, 1946
　　C. Those who have had Address in Japan for at least one year from the date of return to permanent residence
　　D. Those who returned to Japan permanently for the first time after April 1, 1961

The required inspection items are as follows.

• Interviews (subjective symptoms, medical history, etc.)
• Physical examination (visual examination, chest hearing examination, abdominal palpation)
• Physical measurements (height, weight, obesity (BMI))
• Blood pressure measurement
• Urine test (sugar, protein, occult blood)
• Blood tests (neutral fat, HDL-cholesterol, LDL-cholesterol, GPT, GPT, gamma-GTP, creatinine, uric acid, fasting blood glucose, hemoglobin A1C)
• Abdominal girth (for those aged 40 to 74)

The selection inspection items are as follows. Selective tests will be conducted based on the judgment of the doctor who performs the Health Checkups.

• Cardiovascular tests (electrocardiogram test, fundus examination)
• Tests such as anemia (hematocrit value, red blood cell count, hemoglobin)

※Medical examination results will be used as statistical data.
※We may use the personal information of the person who will be consulted for health guidance.

Please note that those who have already undergone a medical examination at employee health insurance, which had been enrolled by the day before their 75th birthday, will not be able to take the Yokohama City Health Checkups this year.
※The fiscal year is from April 1 to March 31 of the following year.

Those who have moved out of Yokohama City and are admitted to a facility that falls under the Address area exception, and have a certificate of insured medical system for the elderly in the latter stage issued by Kanagawa Prefecture, You can use the subsidy system.

■Requirements for the target person (those who can receive the grant)
　Those who meet all of the following requirements are eligible.
　・Being insured person qualification of Kanagawa Prefecture Senior Citizens' Medical Association
　・Having a Address outside Kanagawa Prefecture (subject to the Address Special Exception System)
　・Have not been consulted with Health Checkups in Yokohama in the same fiscal year
