As one of the measures against the lifestyle-related diseases prevention, which focuses on cardiovascular diseases such as heart disease and stroke, the Health Checkups will be implemented once a year.

Kanagawa Prefecture Senior Citizens' Medical System with Address in Yokohama City ※１、２
social security recipients who have Address in Yokohama City who are over 40 years old (including those who turn 40 by the end of the fiscal year)
Those who are 40 years old or older (including those who are 40 years old by the end of the fiscal year) who have Address in Yokohama City and apply to the Chinese residual Japanese support benefit system. ※３

However, those who fall under the following are not eligible.

• Those who are in nursing homes, etc. to which The Long-term Care Insurance applies
• Those who have been hospitalized at a hospital or clinics for more than 6 months

※1 Persons eligible for the medical system for elder senior citizens are those who fall under any of the following.
　　Oh, 75 years or older (from the 75th birthday)
　　B. Those who are 65 years old or older and under 75 years old and have a certain degree of disability and have been certified by the Regional Coalition (from the date of certification)
※(2) If you are in a facility that falls under the exception of the Address area outside the prefecture and have a certificate of insured person for the Kanagawa Prefecture Elderly Care System
　　Oh, prior procedures are required if you wish to consult Health Checkups in Yokohama City at a medical institution in Yokohama City.

The required inspection items are as follows.

• Interviews (subjective symptoms, medical history, etc.)
• Physical examination (visual examination, chest hearing examination, abdominal examination)
• Physical measurement (height, weight, obesity (BMI))
• Blood pressure measurement
• Urinalysis (sugar, tanpaku, occult blood)
• Blood tests (neutral fat, HDL-cholesterol, LDL-cholesterol, GOT, GPT, γ-GTP, creatinine, uric acid, fasting blood sugar, hemoglobin A1C)
• Abdominal circumference (for those aged 40 to 74)

Selected inspection items are as follows. Selective tests will be conducted based on the judgment of the doctor conducting the Health Checkups.

• Cardiovascular test (electrocardiogram test, fundus test)
• Tests such as anemia (hematocrit, red blood cell count, hemoglobin)

※The results of the medical examination will be used as statistical data.
※We may use the personal information of the person being consulted for health guidance.

　※In order to affect test results such as blood sugar levels, please undergo a medical examination at least 10 hours after eating (when hungry) on the day.

If you have already received a medical check-up at employee health insurance, which was enrolled by the day before your 75th birthday, you will not be able to receive the Health Checkups in Yokohama this year.
※The fiscal year is from April 1 to March 31 of the following year.

If you have moved out of Yokohama City and are enrolled in a facility that falls under the Address area exception outside the prefecture, and have a certificate of insured person for the medical system for elder senior citizens issued by Kanagawa Prefecture, You can use the subsidy system.

　・Being insured by the Kanagawa Prefecture Senior Citizens' Medical Association
　・Having a Address outside Kanagawa Prefecture (be eligible for the special Address area system)
　・Have not been consulted in Health Checkups, Yokohama in the same fiscal year
