Overview
incurable disease patients who are treated at home and are highly dependent on medical care, such as using a ventilator, are treated according to the circumstances of their caregivers.
If it becomes temporarily difficult to receive assistance at home, you can enter a designated medical institution in the city for a short period of time.
※ You may not be able to use it depending on the availability of beds and the number of applicants.
Targets
Those who meet all of the following conditions (if temporary hospitalization is possible under other systems, the application of other systems will take precedence)
- Those who live in Yokohama City and suffer from diseases (PDF: 404KB) specified by the Comprehensive Support for Persons with Disabilities Act
- Those who need to be kept under medical control at all times and have stable medical conditions, but are equipped with various medical equipment ※1
- The circumstances of the caregiver are being treated at home. ※Those who have temporarily difficulty in receiving assistance at home due to 2
※1 Those who are using a ventilator, those who are undergoing trachea, those who have stomach wax, and need frequent sputum aspiration
※2 Rest of caregivers (respite), illness, injury, hospitalization, childbirth, ceremonial occasions, business trips, school events, etc.
Hospitalization
Medical institutions designated by Motoichi ※You can't specify the room.
Date of use
It is available up to 14 days a year, up to 84 days a year.
Application documents
- Application Form
- Medical treatment information paper (day life time, etc.)
- Medical information report form from family doctor (please ask family doctor to make it.)
- Other required documents (copy of medicine notebooks, etc.)
If you would like to use the service, please consult with your ward office Elderly and Disabled Support Division at least two months before the month of use.
