In the past, only paper reports were made, but from this fiscal year, online reports via the Internet will be possible after being compiled by the medical institution where you are engaged. (* Medical workers who do not work at medical institutions, etc. will be notified in paper.)
　If you live or work in Yokohama City in Yokohama City, and you submit by mail, please submit it to the private book box installed at the Yokohama City Public Health Center. If there are multiple employers, please indicate the main employer and the subordinate employer on one notification form.
