Doctors, dentists, pharmacists


 If you live in Japan and have a license for a doctor, dentist, or pharmacist, you are required to notify the Minister of Health, Labor and Welfare by January 15, the following year of the Address, the place of employment, and the type of work you are engaged in, etc.
 This year (2024) is the year in which the notification is made. In the past, only paper reports were made, but from fiscal 2022, online reports can be made via the Internet after having been compiled by the medical institution where you are engaged. (* Medical workers who do not work at medical institutions, etc. will be notified in paper.)
 If you live or work in Yokohama City in Yokohama City, and you submit by mail, please submit it to the private book box installed at the Yokohama City Public Health Center. If there are multiple employers, please indicate the main employer and the subordinate employer on one notification form.
 In addition, even if you are not working as of December 31, you must submit a notification form.

Public health nurses, midwives, nurses, associate nurses, dental hygienists and dental technicians


 Public health nurses, midwives, nurses, associate nurses, dental hygienists, and dental technicians engaged in work must submit their names, Address, etc. once every two years to the prefectural governor by January 15, the following year of the year.
