Information on receiving and issuing notifications by mail pertaining to health and hygiene-related administrative procedures

Notification acceptance by mail

Reports accepted at public health centers and each ward office are summarized in a list.
Please check the relevant procedures and check each link.

Notes

  • Please send the notification documents in a traceable manner, such as a specific record or simple registered mail.
    (Our city is not responsible for accidents such as loss or damage during the mailing process of application documents.)
  • We may check the details of the notification, so be sure to specify the Phone number that can be contacted immediately.
  • If you need a copy at hand, please write the reply address on the reply envelope, attach a stamp, and send it with 2 copies of registration form (for submission and copying).
  • When the documents arrive, the acceptance date will be the acceptance date (if the agency has been closed, the next opening date will be the acceptance date). Please send it so that the acceptance date is within the deadline for submitting documents.
  • Please check in advance as attached documents or other notifications may be required.

Those who apply for a new application, renewal grant application, or reissuance of a license for a medical occupation, etc.

Only when the applicant (license qualified person) comes to the counter and confirms the identity, we will accept delivery by mail. If you wish to receive it by mail, please bring a document (one of the following) that can confirm your identity.
In addition, about re-grant application, please bring thing with photograph attached.

  • Car driver's license
  • My Number Card (Personal Number Card)
  • employee health insurance, National Health Insurance, Seafarer Insurance, etc.
  • Mutual Aid Membership Card
  • National Pension Plan (Kokumin Nenkin) Notebook, pension Notebook
  • Pension certificate for National Pension Plan (Kokumin Nenkin), Employees' Pension Insurance Seafarer Insurance
  • Certificates of mutual aid pensions and benefits
  • Handbook for the Physically Disabled
  • Identification card of government offices, etc. (with photo attached)
  • Company ID (with photo attached)
  • Student ID card (with photo attached)
  • Passport (with photo attached)
If you wish to send it by mail, a license to confirm your identity.

Mailing method

It will be sent by proof of delivery (registered mail or letter pack plus depending on the documents).
※Envelopes are marked as "two-fold strictly prohibited", but please note that they may be folded at the time of mailing.
