Tonic health foods and sildenafil (so-called biasgras)

 While there is a problem that "dangerous drugs" are sold mainly on the Internet, tonic health foods of unapproved and unlicensed drugs are also mainly on the Internet, and despite health hazards. Similar products are on sale. Pharmaceuticals mixed in tonic health foods are mainly male hormones (methylteststerone) yohinbin and local anesthetic (lidocaine), and recently syldenafiles.
 Syldenafiles are substances that have a pharmacological effect similar to syldenafil (so-called biasgra). The first approved drug is syldenafil citricate (biagra), which is originally used as a treatment for men's sexual dysfunction. It is also called a PDE5 inhibitor because the pharmacological action inhibits phosphodisterase5 (PDE5), an enzyme that decomposes cyclic GMP. These PDE5 inhibitors have been researched and developed one after another, and hydrate of Valdenafil hydrochloric hydrochloride (Levtra) and Tadarafil (siaris) have been approved as drugs. Recently, tonic health foods (tablets, capsules, drinks) containing syldenafiles have been found one after another, and have become a problem.
 Inspection of tonic health foods at our place revealed that a freearaphyll of 20 mg (one siaris) or more was detected in one capsule (see photo).

 This sample was sold on the Internet, and it was reported that it was also detected in Tokyo and Saga prefectures, respectively. Since one of them was imported and solder in Yokohama, the Medical Safety Division ordered this company to collect products. In addition, as a new problem, the number of products that have similar chemical structures and contain substances that have a tonic effect, such as Syldenafil, Baldenafil, and Tadaraphyll, have also increased (see figure for the structural formula of each substance). These substances are not pharmaceuticals, so care must be taken because safety or toxicity have not been investigated.
 The analysis of syldenafils is mainly confirmed and quantified with a liquid chromatographic mass spectrometer (LC/MS). However, since there is no standard product for analysis of substances with a similar structure to the Syldenafil, etc. mentioned above, the structure of the substance must be determined. Information on the MS spectrum alone is not enough for structural determination, and requires nuclear magnetic resonance (NMR) spectra, gas chromatographic mass spectrometer (GC/MS), infrared absorption (IR) spectra, and elemental analysis.
 Sidenafiles have been reported to have headaches, hot flashes, visual impairment, etc. The combination of syldenafil and nitric acid agent or nitrogen monoxide (such as nitroglycerin) for the treatment of angina may cause blood pressure to drop excessively, cardiac arrest, and death cases have occurred. If you are using nitrate for angina, do not take biasgrass and avoid abuse of tonic health foods even if you are not angina. Also, if you want to take biasgrass, it is important to consult a specialist before taking it.
