How to apply for a request for materials
①Log in to the Yokohama City Electronic Application and Notification System
■Yokohama-shi electronic application, report system (outside site) (outside site)
※New registration is required when using the Yokohama City Electronic Application and Notification System for the first time. Please be sure to write down the ID and password obtained at the time of registration.
②Search for the procedure and proceed to the application form.
From the list of procedures (for individuals), please search for "Yokohama Municipal Hospital Nursing Staff Document Request Application" or access from the following URL.

③Enter all required items in the application form to complete the application.
Make sure that the registered e-mail address has received an e-mail notifying you of your application. You can also check the application status from My Page of the Yokohama City Electronic Application and Notification System.
