How to apply for a document request
①Log in to Yokohama City Electronic Application and Notification System

※When we use Yokohama-shi electronic application, report system for the first time, new registration is necessary. Be sure to write down the ID and password obtained at the time of registration.
②Search for the procedure and proceed to the application form
From the procedure list (for individuals), you have you search for "Yokohama City Hospital Nursing Staff Document Request Application" or access from the next URL.

③Fill out all required items in the application form and complete the application.
