What is the Dementia Initial Intensive Support Team (Kanazawa Orange Support Team)
In order to continue living in a good environment in the area where you are accustomed to living as much as possible even if you have dementia, we will support the team for the purpose of immediate discovery, early diagnosis, and early response.
Visit your home and provide comprehensive and intensive care (approximately 6 months) support for consulting specialized medical institutions and using nursing care services.
Team members
Nurses and mental health workers with dementia specialists and specialized knowledge.
Kanazawa Ward has a team atclinics or Mariya.
Those who are eligible for support
Those who are 40 years old or older, living at home, and have suspected dementia (or dementia) and fall under any of the following.
・Those who do not receive medical services or nursing care services (or have interrupted)
・Those who receive medical services and nursing care services, but have severe symptoms due to dementia and are having difficulty responding
If you would like to use the Dementia Initial Intensive Support Team, please consult your local community care plaza or Kanazawa Ward Office Elderly and Disabled Support Division.
