Diaper charges for people who have been bedridden for more than 6 months (paper diaper purchase costs and rental diaper rental fees) are subject to medical expenses deduction if provided by the family doctor. If you receive the medical expenses deduction for diaper charges after the second year, you can file a medical expenses deduction using the "family doctor Statement of Content Confirmation Form" issued by the ward office Elderly and Disabled Support Division, which certified as requiring long-term care.

Required for the first time declaring medical expenses deduction for diaper charges.
As it is written by your doctor, please hand it to your medical institution.

Please submit it to the Kanagawa Ward Office Elderly and Disabled Support Division (3rd floor of the annex).

The Long-term Care Insurance operators who meet the following requirements may refer to the Kanagawa Ward Office about the progress of the application for The Long-term Care Insurance certification.
１. When information is required for service use, planning and billing.
２. When the relationship between the insured person and the business operator can be confirmed. (Submit application or home notification, etc.)
