2024 Izumi Ward Public Awareness Survey
In order to refer to the various initiatives and projects of the Izumi Ward government office, including the attractiveness of Izumi Ward as a place of residence, the formulation of the 5th Izumi Ward Community Health and Welfare Plan, and the observation of aging changes in residents' awarenessIzumi Ward. This program covers 3,000 inhabitants aged 18 and over who live in Izumi Ward randomly selected from the Basic Resident Register. When you receive the questionnaire, we would appreciate your cooperation.

Investigation method
We will send you a survey form by mail. Please reply to the completed questionnaire or answer from the electronic application system.

Please enclose the questionnaire in the enclosed reply envelope and mail it to you.
