- If you are a family member, please contact Izumi police department first (Tel: 045-805-0110).
- Next, please contact the Izumi Ward Office Elderly and Disabled Support Division or the Regional Comprehensive Support Center.
- The ward office or Regional Comprehensive Support Center that has been contacted requests cooperation organizations (public institutions, transportation, etc.) for cooperation.
- The cooperating organization does not conduct search activities, but will contact Izumi police department if it is found in daily work.

This is a system in which personal information is transmitted to the cooperating organizations via the Izumi Ward Office Elderly and Disabled Support Division (Izumi Ward Office) and the Regional Comprehensive Support Center to cooperate with the discovery during normal operations. It will be used at the same time as the temporary location notification to police department.

- First, please submit a “Temporary Location Notification” to Izumi police department.
- Next, please submit the “Application Form” to the Izumi Ward Office or your local Regional Comprehensive Support Center. You can call me in case of emergency. (You can use it even if you have not registered in advance.)
Please submit "Registration Abolition Notification" to Izumi Ward Office or Local Elderly Care Management Center.
It will not be used for any other purpose. We will manage appropriately in accordance with the Personal Information Protection Law. Pre-registration information will be managed by the Izumi Ward Office, Izumi police department, and Regional Comprehensive Support Center. Information on the day of missing will be sent by fax to the cooperating organization.
