Promotion of comprehensive community care in Isogo Ward

In the area where the elderly are accustomed to living as much as possible, care, medical care, nursing care prevention, life support, and housing are provided in an integrated manner so that they can continue their own life until the end of their lives. In March 2022, we revised the "Isogo Ward Action Guidelines for Establishing Yokohama-based Comprehensive Community Care System" formulated in March 2018.
Reflecting the progress of initiatives since the formulation of the Ward Action Guidelines, and incorporating specific initiatives to respond to new issues, etc., the name was changed to `` Isogo Ward for Construction of a Yokohama-type comprehensive community care system Action Plan ".

Based on the Yokohama City version of the "Action Guidelines for Construction of Yokohama-type Community-based Comprehensive Care System" formulated in March 2017, we have formulated action guidelines for Yokohama City's 18 wards.
In March 2018, Isogo Ward formulated the Isogo Ward Action Guidelines for Establishing Yokohama Community-based Comprehensive Care System.
