Comprehensive Community Care System and Hodogaya Ward Action Plan
With the community-based comprehensive care system, for the baby boomers to reach the age of 75 in 2025, the elderly can live their own lives in areas where they are accustomed to living as much as possible in response to increasing nursing care and medical needs and issues. This is a comprehensive support and service provision system for each area of daily life in which nursing care, medical care, nursing care prevention, life support, and housing are provided integrally so that can be continued until the end.
In Hodogaya Ward, the ward, the Japan National Council of Social Welfare and Community Care Plaza examined the current situation and issues of the ward, their goals, and their efforts, and exchanged opinions with local residents such as medical and welfare institutions, nursing care providers, volunteers, etc., and formulated the Hodogaya Ward Action Plan in March 2022 to build a comprehensive community care system.

In order for all elderly people to live on their own, based on the common philosophy of the Hodogaya Ward Community Welfare and Health Plan "Relieved Town Development", "A town where people can connect and support each other and live with peace of mind." Let's proceed.
