Air pollutant emissions survey

In order to confirm compliance with regulatory standards, Yokohama City conducts air pollutant emissions surveys at factories and business sites that have installed soot and smoke generation facilities every year.

For factories and business establishments in the city where soot and smoke generation facilities are installed, "Air pollutant emission survey form (hereinafter referred to as" survey form "). Please fill in required items and submit it.

It turned out that there was a mistake in the formula of the questionnaire easily, and that the numerical value was not displayed correctly in certain situations. I uploaded the modified Excel again. (June 10, 2024)

We have released an Excel format "Easy Survey Form" that allows you to easily calculate the required form for filling out the questionnaire, so please use it.
