Registration Procedure (update)

 Please submit required documents to the Disability Facilities Service Section by the 15th of the previous month of the registration validity period.
 (Example) Date of registration validity period: December 31, 2023 → Date for submission of renewal documents: November 15, 2023

 When you abolish business registration, please submit "business abolition, suspension, reopening registration form (rules third style)" within ten days from day of abolition.

 If there are any changes to the registered items, please submit "Registration Change registration form (Rule No. 2 Form)" within 10 days from the date of the change.
 Please attach a document that shows the details of the changes.

 Please contact the person in charge for individual consultation. → Please email me. kf-tanichi@city.yokohama.jp
 In addition, about new designation, please assume that it takes about six months after consultation in good condition.

Billing and error

Billing

 Please make a request to the Kanagawa National Health Insurance Federation (National Health Insurance Federation) using the Kanagawa System on the 1st to 10th every month

Please note that payment will not be made in the following cases.
 ● If there is a defect in the contents of the claim, etc., or if the request has not been made
 ● If there is a defect in the contents of the performance record slip, or if the performance record slip was not submitted
 ● If there is a discrepancy between the billing content and the performance record slip
 ● When there is a discrepancy between the billing details and the performance record slip and the target person information

※If you reclaim a claim that has not been paid, such as rejection or unapproved, after the next month, you will need to make a claim to the Kanagawa National Health Insurance Federation (National Health Insurance Federation) using the "Kanagawa System". (1st to 10th of every month)

[Deadline]
 Until 23:59 the day before the last opening day of the month (Example) In the case of October 2023, the deadline is 23:59 on October 30.
 ※If it coincides with Saturdays, Sundays and holidays, the deadline will be moved forward.
 ※It is from December 29 that Yokohama City will be closed at the end of the year. (Example) If December 29 is Sunday, the deadline is 23:59 on December 26.

 24 hours in principle

※If you want to reclaim after the next month due to an error, you will need to make a claim to the Kanagawa National Health Insurance Federation (National Health Insurance Federation) using the Kanagawa System. (1st to 10th of every month)
