Accreditation of transport business for patients and others

This is a document pertaining to the application for transportation business certification for patients. Documents to be submitted to the Fire Bureau Emergency Planning Division.

It is document to submit when transport companies such as patients with offices in Yokohama City are going to receive business certification of City of Yokohama, Fire Bureau. In addition, please submit this document when company which received business authorization performs update procedure of certification.

When we receive business authorization or perform update procedure, we first style (documents to submit with transport business authorization such as patients, update application. When submitting, it is necessary to submit a copy of the certificate of eligibility for transport crew such as patients of each crew member.

This is a document to be submitted along with the transportation business certification for patients, renewal application, and crew list when trying to obtain City of Yokohama, Fire Bureau's business certification.
