This document is required when taking a lifesaving course or an introductory lifesaving course.

Emergency transport
This document is required to prove the contents handled by the emergency services.
Request for Emergency Certification (word: 30KB)
This document is required to prove the location and name of the rescue squad, the date and time of the request, and the location and name of the medical institution that was housed. Please submit it to the fire department to which the rescue squad belongs.
Patient transport business certification
This is a document pertaining to the application for certification of transport business such as patients. Documents to be submitted to the Fire Bureau Emergency Planning Division.
This document is to be submitted when a transport company such as a patient with a business office in Yokohama intends to obtain a business certification from City of Yokohama, Fire Bureau. In addition, please submit this document when company which received business authorization performs update procedure of certification.
No. 2 style (cay crew list) (word: 49KB)
When we receive business authorization or perform update procedure, it is document to submit with the first style (transportation business authorization such as patients, update application. When submitting, it is necessary to submit a copy of the certificate of eligibility for transport crew such as patients of each crew.
No. 3 style (report of car for transportation such as patients) (word: 49KB)
Documents to be submitted along with the certification of the transport business for patients, etc., renewal application, and crew list when attempting to obtain City of Yokohama, Fire Bureau's business certification.
