Request the creation of a preventive care plan at the Local Elderly Care Management Center (Community Care Plaza) that is in charge of your area.
Care managers of the Local Elderly Care Management Center set goals for users, incorporate services for care prevention, and create a care prevention care plan.
How to use the service (for those requiring support 1.2)
