This meeting is for families of care recipients who are cared for at home. Why don't you talk about friends who are struggling with nursing care, experiences?
The burden on the mind is also reduced. In addition, we hold study sessions and other events. The venue will be at Care Plaza. Please contact us for the date and time, venue and details.
What is the Regional Comprehensive Support Center?
We will set up a “Local Elderly Care Management Center” at the Community Care Plaza, which provides various support for making good use of The Long-term Care Insurance and other services so that the elderly can continue living in the area where they live.
The "Local Elderly Care Management Center" has specialized staff such as public health nurses, social workers, and chief care managers, and provides various consultations and care plans for preventive care.
Role of Regional Comprehensive Support Center
- A series of "care prevention care management" that creates a care plan for those who need support 1 and support 2, and create a care plan (*) for those who may need support or need nursing care (*).
※Care plan creation is limited to community care plazas in charge of the target area (see table below). - "Comprehensive consultation desk function and advocacy function" that leads to various consultations and provides necessary services
- "Comprehensive and continuous care management support" such as coordination with ward offices and related organizations, and support for care managers
