This network is a system that enables elderly people with dementia (including persons with disabilities over the age of 18) to confirm their identity and contact their families smoothly when they go missing. The ward office, community care plaza, police department and transportation are working together.
Pre-registration
You can register information (physical characteristics, photos, etc.) of people with dementia who may be missing at ward offices or community care plazas in advance.
■Target person
1.Living at home in Naka Ward (excluding those who are admitted to facilities or long-term hospitalization)
2.There are family members who can pick you up
■Belongings
○Photographs (Recent ones that clearly show the face of the person)

Pre-registration information is stored and managed at Naka Ward Office Elderly and Disabled Support Division, Community Care Plaza and police department.
Flow when missing
■If you realize "No,"
1.If you are a family member, first request a search from police department or "Koban" (police box) near the missing location.
2.Next, please contact the organization (ward office, community care plaza, etc.) that applied for registration of the SOS network.
3.The registered organization that has been contacted requests cooperation organizations (public institutions, transportation, etc.) for cooperation.
4.The cooperating organizations will respond in their daily work.

1.The discoverer will contact the ward office or community care plaza and will contact your family from here.
2.If you are a family member, please report to the organization that applied for SOS network registration that you have returned.
Yokohama City Dementia Elderly Watching Seal Business
In Yokohama City, we distribute a "watching sticker" that can identify the identity while protecting personal information so that people with missing dementia can return to their homes early. We distribute two types of sticker type (36 sheets) and iron sheet type (34 sheets) as a set so that it can be used on clothes and belongings.
