(2) SOS Network Business for Elderly People with Dementia in Naka Ward / Yokohama City

This network is a system that allows elderly people with dementia (including persons with disabilities aged 18 and over) to confirm their identity and contact their families smoothly when they are missing. The ward office, community care plaza, police department, transportation, etc. are working together.

Pre-registration

 We can register information (physical characteristics and photographs) of people with dementia who may be missing at ward offices or community care plazas in advance.

 ■Target person
 1.Living at home in Naka Ward (excluding those who enter facilities and long-term hospitalization)
 2.There are family members who can pick you up

 ■Belongings
 ○Photographs (a recent photo showing the person's face clearly)
 ・One full-body photo
 ・1 face photo
 ○emergency contact number
 ※At the time of registration, please provide contact information and usage status of The Long-term Care Insurance service.

 ■Procedure
 ○Naka Ward Office Elderly and Disabled Support Division (5th floor, 51st floor)
 ○Community Care Plaza in Naka Ward
 ○Naka Ward Living Support Space for Persons with Disabilities (only for persons with disabilities)
 ○Community Activities Home for Persons with Disabilities in Naka Ward
 ※Reception hours, please refer to the SOS Network for Elderly People with Dementia in Naka Ward.

 ■About the registered information
 Pre-registration information is stored and managed at Naka Ward Office Elderly and Disabled Support Division, Community Care Plaza and police department.

Flow when missing

■If you notice "No."
 1.Family members should first ask the police department or "Koban" (police box) near the missing location.
 2.Next, please contact the organization that applied for the SOS network registration (such as a ward office or community care plaza).
 3.The registered organization that receives the notification requests the cooperating organization (public institution, transportation, etc.) for cooperation.
 4.Cooperating organizations will respond in their daily work.

■If the person is found
 1.The discoverer will contact the ward office or community care plaza, and from here to family members.
 2.If you are a family member, please report that you have returned to the organization that applied for the SOS Network.

Yokohama City Elderly People with Dementia Watching Seal Business

 Yokohama City distributes a "watching sticker" that allows you to identify yourself while protecting your personal information so that missing dementia can return to their homes early. Two types of seal types (36 sheets) and iron sheet type (34 sheets), are distributed as a set so that they can be used on clothes and belongings.

■Target person (elderly people with dementia corresponding to all of the following 1 to 4 (including those with juvenile dementia))
 1.A Yokohama citizen
 2.Living at home (excluding those who are admitted to facilities and long-term hospitalization)
 3.Pre-registered SOS Network for Elderly People with Dementia, and agreed to provide registration information to Kanagawa and Kanagawa Prefectural Police        I am doing it
 4.There are family members who can pick you up
