Naka Ward's comprehensive community care system

What is a community-based comprehensive care system?

The community-based comprehensive care system is a system that helps each other in the community so that they can continue their own life in the area where they are accustomed to living, even if they need support and nursing care. Yokohama City is working on the construction of a "Yokohama-type community-based comprehensive care system" that promotes detailed approaches according to the characteristics of each area of daily living.
Naka Ward is also working with related organizations and professionals to create a town where the desire to continue living in their own Naka Ward, where they are accustomed to living!

Characteristics of Yokohama-based comprehensive community care system

  1. We promote approach mainly on "community care plaza" which is base of welfare health, local interchange.
  2. In cooperation with active citizen activities such as NPOs and volunteer activities, we develop various services.
  3. Aiming to be the best health and longevity in Japan, we will work on health promotion and nursing care prevention.

Naka Ward Action Plan (formerly the Action Guidelines) for the construction of a comprehensive community care system

In March 2018, Naka Ward formulated the Naka Ward Action Guidelines for Establishing a Comprehensive Community Care System in Yokohama.
Based on the efforts since the formulation of the Action Guidelines and changes in social conditions, it has been revised as the "Naka Ward Action Plan for Construction of a Yokohama-type Community-based Comprehensive Care System" with a planning period until Reiwa 7 (2025).

We are promoting the creation of a system in which medical, nursing, and welfare workers can work together beyond their affiliations so that they can live with peace of mind in the area where they are accustomed to living even if they need medical care or nursing care.
Even if you want to be greeted at home at the end, "Can I get medical care at home?" Isn't there a lot of uneasy or unknown things, such as "Where should I consult?"
In order to convey information about home medical care to residents in an easy-to-understand manner, Naka Ward plans and edited with medical and nursing care professionals to convey information about home medical care to residents of the city in an easy-to-understand manner.
