About the comprehensive community care system in Naka Ward

What is a comprehensive community care system?

The community-based comprehensive care system is a system that helps each other in the community so that they can continue their own life in the area they are accustomed to even if they need support and nursing care. In Yokohama City, we are working on the construction of a “Yokohama-style comprehensive community care system” that promotes detailed initiatives according to the characteristics of each area of daily life.
In Naka Ward, we are working together with related organizations and professionals to create a town where the desire to continue living like oneself in Naka Ward is fulfilled.

Features of Yokohama-type comprehensive community care system

  1. We promote approach mainly on "community care plaza" which is base of welfare health, local interchange.
  2. We will develop a variety of services in cooperation with active citizen activities such as NPOs and volunteer activities.
  3. Aiming to be the best health and longevity in Japan, we will work to promote health and prevent nursing care.

Naka Ward Action Plan (formerly Action Guidelines) for the construction of a Yokohama-style comprehensive community care system

In March 2018, Naka Ward formulated the Naka Ward Action Guidelines for the Establishment of a comprehensive community care system for Yokohama, clarifying its goals and working to build a comprehensive community care system.
Based on the efforts since the formulation of the Action Guidelines and changes in social conditions, we have revised the Naka Ward Action Plan for the Construction of a Yokohama-style comprehensive community care system with a planning period of 2025.

We are promoting the creation of a system in which medical, nursing, and welfare workers can cooperate across affiliations so that they can live with peace of mind in the area where they live even if they need medical care or nursing care.
Even if you want to meet at home at the end, "Can you get medical care at home?" There are many things you are worried or don't understand, such as "Where should I consult?"
In Naka Ward, in order to convey information about home medical care to residents in an easy-to-understand manner, the leaflet "Tell me! "Home Health Care" has been created. Please take a look.
