About questionnaire findings to devise the fourth Minami Ward community-based welfare health plan

Minami Ward is currently considering the formulation of the 4th Minami Ward Community Health and Welfare Plan (scheduled for FY2019 3-7). For development, we carried out questionnaire survey to inhabitants of a ward and related organizations to grasp the way of thinking about the community and issues related to welfare and health.

（１） Inhabitant of a ward questionnaire survey
A. Survey period: From May 27 to June 21, 2019
(b) Target: 3,200 men and women aged 20 and over living in the city (random sampling)
C. Investigation method: Mailing (distribution and collection)
D. Recovery results: Number of questionnaires sent (3,200), number of collections (1,601), and collection rate (50.0%)

（２） Questionnaire survey of related parties
A. Survey period: From May 31 to June 28, 2019
(b) Target: Approximately 350 organizations, including the Association of Neighborhood Associations, District Children's Association, Community Shakyo, Welfare Facilities and Volunteer Organizations
C. Investigation method: Mailing (distribution and collection)
D. Recovery results: Number of survey forms sent (329 groups), number of collections (253 groups), collection rate (76.9%)
