At the window of the ward office, the following procedures are mainly available.

1. Reissue of pre-examination vouchers
2. Procedures for being vaccinated outside of Yokohama City (If you are returning home or being hospitalized at a medical institution outside the city, you can also apply electronically. Please see this (Yokohama City Electronic Application Form) (outside site). )
3. Issuance of English Certificates
4. Others (questions and consultations regarding Vaccinations)

In any case, be sure to bring your personal identification documents such as the Maternal and Child Health Handbook and Health Insurance Card (for the recipient and the proxy/agent who can come to the procedure).
