What is the SOS Network for Elderly People with Dementia Konan Ward
Depending on the symptoms of dementia, you may get lost because you don't know where you are on the go or where your home is.
Over a long period of time, the risk of dehydration, traffic accidents, and injuries caused by falls increases.
The SOS network is a mechanism to detect and protect elderly people with dementia as soon as possible when they go missing. The ward office and police department, community care plazas and public transportation will work together to ensure that elderly people with dementia and their families continue to live in a familiar area with peace of mind.
To all family members (to supporters)
By pre-registering the characteristics and photos of the person, when missing, you can request a cooperating organization such as transportation to discover cooperation based on the registration information. Pre-registration can be done at the ward office or community care plaza.
You can receive the application form at each window or download it from this page.

●Registration application form (you can hand it over at the counter)
●2 photos showing the face of the person (upper body photo, whole body photo)
●emergency contact number (up to three locations including applicants)

About change, abolition of registration contents
If your situation changes and you do not need to register, such as changing the Address or contact information of the person or emergency contact number, moving out of the ward or entering a facility, please notify us of the change or abolition. (The paper can also be handed over at the counter.)

If you see such an elderly person…
If you see a person who is worried about, such as seasonal clothes, Chiguhaguna footwear, pajamas, or your belongings with a name tag or contact information, please watch the situation casually first.
Then gently say, "Where are you going out?"
If you don't know your home or where to go.…
Please contact your name tag or the police. Please keep someone close by you until the pick-up person comes.
At first glance, even if it looks firm, you may go out again.
You may have been walking for a long time, so recommend a break or drink.
As calm as possible. Please try to touch it slowly.
