Please send "Questionnaire (Form B) (word: 19KB)" to Konan Ward Health and Welfare Division Business Planning Officer by e-mail (kn-cp@city.yokohama.lg.jp) or fax (045-846-5981). In addition, please confirm receipt by telephone (045-847-8441) after sending.

On Friday, January 24, 2025 (planned), we will announce the answer on this website.
