What are the certifications?

 Various certifications are proofs issued as administrative services without being based on laws and regulations.
 Certificates are issued based on confirmation materials such as ledger information stored in the ward Health Sanitation Division.

[Common] What is required for application

 ・Certification application form (No. 8 style) ※I will hand it over at the counter.
 ・Documents (power of attorney, employee ID, etc.) that confirm the identity of the applicant (business operator) or proxy/agent, etc.
 ・Costs (fees) 300 yen per copy ※Cash only

Certificate of application for business license etc.

What is the proof of application for business license etc.?

 When a certification is applied during the period from the application date to the disposal decision date, the contents of the application form will be confirmed for the following items and a certificate will be issued.
 ・Applicant Address, applicant name, name of business facility, location of business facility, type of business, date of application

 Applicant for permission of the facility to be certified

Certificate of entries in the ledger

What is the certificate of entry in the ledger?

 As the current items at the time of application for certification, the contents of the business ledger are confirmed for the following items and a certificate is issued.
 ・Business operator Address, business operator name, business facility name, business facility location, business type, date of permission, etc.
 ・(Public baths only) The presence or absence of administrative sanctions and, if so, its contents

Notes

 If there is a change from the contents of the business ledger, please apply for certification after notification of the change.
 We will issue a certificate with the content that reflects the changes in the notification of change.

Certificate of abolition notification

What is a certificate of abolition notification?

 In the event of an application for certification during the period from the date of notification of abolition to the deletion of the ledger, registration form will confirm the contents of the following items and issue a certificate.
 ・Business operator Address, business operator name, business facility name, business facility location, business type, date of permission, etc. number, date of notification of abolition
