What are the certifications?
Various certifications are proofs issued as administrative services, not based on the grounds of laws and regulations.
We issue certificate based on confirmation materials such as ledger information stored in ward Health Sanitation Division.
[Common] What is required for application
・Certification Application Form (No. 8 Form) ※You will receive it at the counter.
・Documents (a power of attorney, employee ID, etc.) that can confirm the identity of the applicant (business person) or proxy/agent
・Cost (fees) 300 yen per copy ※Cash only
Proof of application for business license etc.
What is the proof of application for business license etc.
If a certification application is made during the period from the date of application to the date of disposal, the contents of the application form will be confirmed for the following items and a certificate will be issued.
・Applicant Address, Applicant's name, business facility location, business type, date of application
A person who can apply
Permit applicant for the facility to be certified
Proof of entry in the ledger
What is the Ledger Certificate?
As the current matter at the time of application for certification, the contents of the business ledger are checked for the following items and issues a certificate.
・Operator Address, name of business operator, business facility location, business type, date of permission, etc. Number
・(Public bath only) With or without administrative sanctions, if there is, its contents

Precautions
If there is a change in the contents of the business ledger, please apply for certification after notification of the change.
We will issue a certificate with contents reflecting the changes in the notification of change.

Proof of abolition notification
What is the certification of abolition?
If an application for certification is made during the period from the date of notification of abolition until the deletion of the ledger, the contents of registration form will be confirmed for the following matters and issue a certificate.
・Address, name of business operator, name of business facility, location of business facility, business type, date of permission, etc., license number, date of notification of abolition

Notifier of the facility for which certification is to be certified
