About procedure of establishment, change of beauty salon
When performing treatments related to hair (cuts, colors, etc.), makeup, and eyelash extensions, it is necessary to confirm the conformity of the beauty salon.

Flow until the establishment of a beauty salon
1 Prior consultation → 2 Notification of establishment → Inspection of 3 facilities → 4. Issuance of Conformity Confirmation and commencement of business
1 Prior consultation
In order to receive conformity confirmation, the facility must conform to the standards.
Before constructing the facility, please bring a drawing that confirms that it conforms to the standards of the facility and consult with Kohoku Health and Welfare Center.
Also, check with the operator to see if the facility meets the standards.

2 Notification of establishment (submission of documents)
Please submit the following documents about 1-2 weeks before the desired date of opening.

・Opening beauty salon registration form (first style) (word: 24KB) ・ 1 copy ★Example of description (PDF: 245KB)
・Floor plan of the facility (fill in the layout and dimensions of structural equipment, etc.) ・ 2 copies
・A sketch of the neighborhood ・・ 1 copy
・Hairdresser's license [original] ※After confirmation, I will return it. ・・ For all hairdressers engaged
・Medical certificate of doctor about tuberculosis, presence or absence of skin disease (within 3 months after diagnosis) ・ For all hairdressers engaged
・If there are two or more hairdressers, a certificate of completion of the management hairdresser training course [original] ※After confirmation, I will return it.
・If the establisher is a corporation, a certificate of entry (within 6 months from issuance) [original] ※After confirmation, I will return it.
・If the establisher is a foreigner, a copy of resident certificate (with nationality, etc.) ・・ 1 copy
・Fee 16,000 yen (cash)
Inspection of 3 facilities
After the facility is completed, a field survey will be conducted to confirm that it conforms to the facility standards.
※As a result of the survey, if the facility standards are not met, a resurvey will be required.
4. Issuance of Conformity Confirmation and commencement of business
After the facility inspection, a confirmation of conformity will be issued in about one week. It can be delivered at the counter or by mail.

Notification of change after establishment
In the following cases, a change notification is required.
・When employees are changed (entering, leaving, etc.)
・When the company representative, business operator Address or facility name is changed
・When facilities are changed (change of the number of set chairs, change of operating area, etc.)
※When changing facilities, please consult with Kohoku Health and Welfare Center before starting construction.
