When visiting the office, please cooperate with the following points.

• If you have a cold, a fever of 37.5 degrees or higher, or an acute rash, please refrain from consulting.
• When visiting the office, please cooperate with the prevention of infection such as temperature measurement in advance and hand disinfection.
• Please refrain from consulting not only for children who are subject to medical examinations but also for those who have symptoms such as fever or cough in their family.
• Thank you for your cooperation in taking out used diapers. Please bring a filth bag.

• On the day, the parking lot and surrounding roads will be very crowded. Please use public transportation (train, bus) as much as possible to the next agency.
• Because the inside of the medical examination hall is narrow, you will not be able to take a medical examination while keeping it on a stroller. If necessary, please bring a baby strap. In addition, there is a stroller storage area at the entrance, so please use that. Also, if you are pregnant or bring your siblings and wish to use a stroller, please contact the reception staff on the day.

• Please bring your mother and child handbook, urine and urine test application form.
• Please be sure to submit it to the medical examination site on the first floor of the Kohoku Ward Office in Reception hours on the day of the medical examination for children aged 3 years.
• I confirm that I have visited the Kohoku Ward Office. If you cannot confirm your mother and child handbook, you may not be able to be tested.
• If you can't collect urine on the day, you don't need to bring it with you. Also, by the day before your 4th birthday, you can take it in the Reception hours, the next 3-year-old child's health check-up. Details will be announced at the reception desk on the day.
