Kohoku Ward Check ups for Infants and Toddlers Reservation Change Page

This page is for those who have received the Check ups for Infants and Toddlers schedule (mail) from Kohoku Ward Office Children and Families Support Division and are unable to visit the agency on the schedule.
You do not need to apply if you can come according to the schedule stated in the notice.

If you submit a urine test for a 3-year-old Health Checkups at a later date, you do not need to make a reservation.
Please come to Reception hours.

[Procedures]

1. If you click on the application form for each medical examination, you will be connected to the external site Yokohama City Electronic Application System.
2. Please select the desired schedule in the application form. (If it is full, you will not be able to select it.) Please consider a different schedule.
3. When the application is completed, you will receive an application completion email.
4. Please come to the agency as it is on the day.
5. If you make a reservation more than two months away from the date of the announcement, Children and Families Support Division may contact you with the listed Phone number. Thank you for your understanding.

To cancel your reservation, please withdraw your application from My Page of the electronic application system.
[How to cancel a reservation from My Page]

1. Log in to My Page of Yokohama City Electronic Application System
2. Click the list of application history and search.
3. Click the application you want to withdraw.
4. Go to the details screen
5. Click Delete this application.

If you wish to change the reservation you have applied for once, you must cancel the original reservation once as described above.
We ask you to make a new reservation.

The reservation page differs depending on the age of the month and the year of consultation. Please be careful not to make a reservation.
