Individual consultation by public health nurses, dietitians, and dental hygienists
・physical checkup's inspection figures and results data
・About diet, exercise, oral care for prevention and improvement of diabetes, hypertension, dyslipidemia, obesity, etc.

・Examination numerical values such as the results of medical examinations and those that understand the status of medication (if you have one)
・Memo of meal contents (record and bring as much as possible)
