Ward Office 5th floor Health Consultation Room

Individual consultation by public health nurses, nutritionists, and dental hygienists
・physical checkup's inspection figures and results data
・About diet, exercise, oral care, etc. for prevention and improvement of diabetes, high blood pressure, dyslipidemia, obesity, etc.

・Examination numerical values such as medical examination results, and medication status (if you have one)
・Memo of meal contents (Please record and bring it as much as possible.)
