When making a reservation, the staff in charge of the elderly support will ask you about the situation.

Due to cognitive decline, you may not know where you are now, where your home is, and you may get lost.
By registering in advance, the SOS network, when the person with dementia goes missing, the ward office, community care plaza, police, and cooperating organizations will share information and return to the family as soon as possible. It is a mechanism to cooperate so that you can return safely.
In addition, to those who have been registered in advance, we distribute a "watching sticker" that allows you to identify their identity while protecting personal information.

1.Photo of the person (three face photo and whole body photo, the same one in color copied is acceptable)
※Please submit the photo without sticking. Please prepare the size of the photo about the L version (127 x 89 mm).

Yokohama City distributes a "watching sticker" that allows you to identify yourself while protecting your personal information so that missing dementia can return to their homes early.
Two types of seal types (36 sheets) and iron sheet type (34 sheets), are distributed as a set so that they can be used on clothes and belongings.

Elderly people with dementia who fall under all of the following 1 to 4 (including those with juvenile dementia)
１．A Yokohama citizen
２．Living at home (excluding those who are admitted to facilities and long-term hospitalization)
３．Pre-registered SOS Network for Elderly People with Dementia, and have agreed to provide registration information to Kanagawa Prefecture and Kanagawa Prefectural Police
４．There are family members who can pick you up
