Please submit a questionnaire with registration form pregnancy at the ward office. A maternal and child health handbook will be issued.
Specifically, please see page of City of Yokohama, Child and Youth Bureau "grant of mother and child health handbook".

It is available at the ward office. If you fill out the form at home in advance, please download and bring it from the link below.
