We provide consultation on welfare and health, accept applications for certification for long-term care in The Long-term Care Insurance, and provide services outside of The Long-term Care Insurance. Social workers and public health nurses are available for consultation.

From April 2006, the Community Care Plaza and Special Nursing Care Center, which provides various support to make good use of The Long-term Care Insurance and other services so that the elderly can continue living in the area where they live. We install in nursing home. The "Local Elderly Care Management Center" has specialized staff such as public health nurses, nurses, social workers, and chief care managers, and provide various consultations and care plans for preventive care.

If you encounter changes in your daily life, such as "It's easier to fall" or "I'm going to go out" in your daily life, we recommend that you start your care prevention efforts as soon as possible. Please feel free to contact the Regional Comprehensive Support Center where you live.
