What is the SOS network for elderly people with dementia in Tsurumi Ward?

(You can use it even if you have not registered in advance.)

What to prepare

We download style from web page or hand in community care plaza, ward office Elderly and Disabled Support Division.

About registration information

The registered information will be kept at the Community Care Plaza and the ward office in charge of your area.
If you wish, you can also register information with the Kanagawa Prefectural Police, which will lead to a quicker search.
※The registered personal information will not be used for any purpose other than the purpose of this project.

When the situation changes from the time of pre-registration

If there is a change in the Address or contact information of the person or the person in emergency contact number, please write down the changes inregistration form (Form No. 2) and submit it to the community care plaza in your area.
If you move out of the ward or enter a facility, or if there is a change in your condition and you no longer need to register SOS, please notify us of the abolition (No. 2 style).
※Once every few years, your community care plaza will contact you to confirm your registration details.

When the person goes missing

①First, please contact Tsurumi police department for a search request.
②Next, please contact the Community Care Plaza or Elderly and Disabled Support Division Tsurumi Ward.
 ※If you have registered in advance, please let us know.
③The community care plaza, etc., which has been contacted, requests discovery cooperation organizations for discovery cooperation. (The discovery cooperating organization will cooperate with the discovery of the person in normal work.)

If you find a missing dementia, you can read the two-dimensional code of the sticker with a smartphone, etc., and contact the discovery information, which will lead to support you to return to your home early .
If you wish to use the service, you can apply according to the registration of "Wanana Net".
