Child care medical care benefits
- ● What is the medical care benefits System for Premature Infants
- This system subsidizes the medical expenses (self-pay) when a premature infant is admitted to a designated nursing care institution.
- ● Targets
- baby/infant weighs 2,000g or less at birth.
baby/infant, regardless of weight, is particularly weak from birth and needs hospitalization. (Depending on the doctor's judgment)
Application for "Subsidies for Medical Expenses for Pediatric Treatment" is also required. - ● About period
- From birth until discharge from hospital. (up to two days before the first birthday)
- ● About hospital
- If you are a designated hospital, you will not be able to use this system. Please contact the hospital to see if you have been designated.
- ● About self-pay
- After the medical ticket is sent, please make an account at the hospital counter guide. Refunds are not available.
There is no self-pay for insurance medical treatment. Please pay only the non-insurance medical expenses (diaper fee, differential bed fee, etc.)
The cost of mother's birth is not paid in child care. - ● About application
- Please prepare the following documents to Children and Families Support Division.
1.Application form: Please fill in your own form.
2.Household record: Please fill in your own form.
3.Statement of Opinion: Please ask me to fill out the form at the hospital. Please check the hospital name and doctor private seal.
4.Income tax amount certificate: For all income household members, one of the following documents is required. (Latest)
A. Withholding tax slip (year-end adjusted)
(A) Copy of tax return (with tax office reception stamp)
(C) City prefectural tax taxation certificate (except for those who have recently been transferred to Yokohama City, you can receive it free of charge at the ward office)
5.Health insurance card, my number, official certificate with photo of your face (license, etc.): You do not need to submit all three items, but please bring them to confirm the items described. - A medical ticket will be mailed in about 3 to 4 weeks after the application.
