Organizations wishing to participate will fax by 5:00 pm on Tuesday, January 7, 2020. ※Please send the application form (Form 12) of the application documents related to each community care plaza to the Naka Ward Health and Welfare Division Business Planning Section by e-mail.
In addition, on the day of the briefing, there is no dedicated parking lot at the venue, so please use public transportation. In addition, please note that on-site tours will not be held without application.
