Types of licenses that can be accepted

Health Sanitation Division accepts applications for new licenses, renewal of registration and renewal, and re-grant applications.

License issued by the Minister of Health, Labor and Welfare

Doctors, dentists, pharmacists, public health nurses, midwives, nurses, medical radiologists, clinical laboratory technicians, sanitary technicians, physiotherapists, occupational therapists, optics, corpse autopsy qualification application, administrative dietitian

License issued by the Governor of Kanagawa Prefecture

Associate nurses (*), conception control on-the-job instructor, nutritionist, cook, drug handler, blowfish knives, confectionery hygienist, cleaning teacher

※Only associate nurses can handle licenses issued by other prefectures.

Those who live in Yokohama City
(For applications for public health nurses, midwives, and nurses' registration, those who have a work location in Yokohama City)

Precautions for Application

In principle, an application by the person who intends to obtain (or already has) a license. If it is difficult to apply, please contact us in advance.
In particular, when applying for a re-grant, applications from anyone other than the person will not be accepted in order to prevent false applications by malicious third parties.
Please bring your driver's license, health insurance card, etc. as a certificate that can identify you and come to the agency.
The fee for the Kanagawa prefectural governor's license will be paid at the financial institution window (there is no bank that can be paid in the ward office). For this reason, please come to the agency with plenty of time so that payment is completed within the business hours of the bank.
Only in the case of application from the person, we will accept the grant (receipt) by mail. (In the case of proxy/agent applications, mail delivery will not be accepted.)
If you would like to receive it by mail, please bring a certificate that can identify you, such as a driver's license or health insurance card, and come to the Agency. In this case, stamps, envelopes, and cardboard (to prevent folding) required for mailing will be borne by the applicant, so please prepare them in advance.
