We will issue certificates to those who volunteer at the Kids Club or After-School Care Program Clubs after school in Minami Ward.

How to apply
Please attach the following documents by electronic application (outside site) or mail and apply.
（１） Request form for issuing a certificate of volunteer activity (please fill in required items)
（２） Diary (fill in the contents and receive the club staff's signature or seal in the club confirmation column) Scan image file or copy
