Volunteer Certificate (After School Kids Club After-School Care Program Clubs)

Those who volunteered at the after-school kids club in Minami Ward or After-School Care Program Clubs

Please attach the following documents by electronic application (outside site) or mail, and apply.

（１） Request for Issuance of Volunteer Activities (Please fill in required items)

（２） Diary (fill in the contents and got the sign of the club staff or seal in the club confirmation column) Image file or copy

Within three months from the date of volunteering

・It will take about 3 weeks after the application.
・The certificate is issued by Children and Families Support Division Minami Ward.
