I lost my private seal registration card.

Please report the loss to the office of your ward office. It is not possible to submit a report at the administrative service corner.
If you have lost your private seal registration card and need Seal Registration Certificate, you will need to report the loss of private seal registration certificate and apply for Seal Registration again.

（１） When the person reports the loss of the private seal registration certificate

• Please bring your personal identification document, private seal.

（２） When proxy/agent reports the loss of private seal registration certificate

• Please bring your registered private seal, Power of Attorney, seal of proxy/agent and proxy/agent.
• When proxy/agent submits a notification, the document inquiry method must be used.

（３） When the person submits a Seal Registration at the same time as the private seal registration certificate

• Please bring your personal identification document, private seal. (If you want to change your registration seal, please bring your new registration private seal.)
• Document inquiry method
  After the reception, an inquiry will be sent to you by mail from the ward office. Please have your own Address, name, and private seal to be registered on the inquiry you received, and come to the agency again.
• Licensing method
  If you present a personal identification document issued by a public office with a photo of your face within 10 years after issuance (driver's license, passport, etc.) as a personal identification document, Seal Registration will be done on the same day.
• Guarantor method
  If a person who operates in Seal Registration, writes Address, name and date of birth in the warranty section of the Seal Registration application form and sealed the registered private seal, then Seal Registration will do so on the same day. If your guarantor is registered as a resident outside of Yokohama, you must attach a Seal Registration Certificate within three months after issuance.

（４） When proxy/agent performs Seal Registration at the same time as reporting loss of private seal registration certificate

• • Please bring your registered private seal, Power of Attorney, seal of proxy/agent and proxy/agent.
  • (If you want to change your registration seal, please bring your new registration private seal.)

• When proxy/agent submits a notification, the document inquiry method must be used.
