• Original insurance card
• Documents that can confirm the chassis number, such as vehicle inspection certificate
• Driver's license

In addition, private seal (when applying under a corporate name, a representative seal is required) and Yokohama City Revenue certificate stamp 750 yen (sold at the ward office) are required.
